Go to the UOP and check it there.
Do not always think that the DOs are configured properly base on their description.
In Cycle DO does not necessarily mean it is linked with the "BUSY" UO.
How to link UO to DO?

Can be linked via flags.
Flags are not required to link them. When on the UO screen press 'config' and see the rack, slot, start assignment. Then see if the DO config matches. If so then they are not so much "linked" as they are mapped to the same item.
Is your UO[3] for program running coming on?
UO(3) is getting ON . But DO(11) incycle not getting on. Manual says it is system generated.

That would mean the DO is not configured to the correct UO.
