We have a brand new Kuka KRC4 with the option Profinet controller/device.
Can we use Profisafe for the safety (connection with S7 1513 F), or do we need an additional option package for failsafe?

In short - yes!Kuka safety interface comes in two flavors:
a) Basic safety
b) Safe OperationBasic Safety is mandatory. It allow you to get state of signals such as External EStop, Operator Safety, External Enable Sw, etc.
Safe Operation is an optional package. It adds safety zones, speed monitoring etc.
With PNET you need to use 8-byte (64-bit) for safety signals.
If you are only using basic safety, first two bytes are all that matters.If you were to use EIP, you could do the same or... if you are only using Basic safety, you can also select 2-byte (just basic) to reduce size of transferred safety data block.

[size=1em]Note, signal map is fixed (and same regardless if using EthernetIP, ProfiNet or EtherCat). Assignment of signals is documented in SafeOp manual. Do not ignore reserved bits, they must be initialized in safety PLC or this will never work.[/size]
