Fanuc NTED SRVO 374 & 375 problem

Installed a Banner NTED on an R30iA controller
Worked great in manual mode. Tried to put the robot in “Auto” mode and got a SRVO 374 & 375 MODE 12 fault
I looked up this error and will be replacing the operator panel per Fanuc tech support. Can someone please tell me what went wrong? The simple wiring was correct.

UPDATE: Before installing the new operator panel, I blew out the old one since the controller is located inside a powder room. I then decided to cycle power one last time and the fault cleared. We have been running two days now without further incident.
At a later time I will test out the NTED again, especially since I have a replacement part should things go wrong.
